Insurance information

For patients with the following health insurance, our office will electronically submit claims to your insurance company, and you are responsible for any co-payment and deductibles at the time of service:

  • The doctor will only recommend medically necessary lab test, radiology studies, and /or medical devices. There are many different health insurance policies with variety of different coverage terms. It is the patient's responsibility to know if he/she has any deductible for such tests or device. The patient always has the option to call his/her insurance company to find out the estimated cost before any testing is done. The doctor's office can help to provide the diagnosis codes and testing codes for the patient to call his/her insurance to find out the estimated expense for the patient before any test, if the patient wishes to make the call.

    Please be aware that patient information sheets and insurance information must be updated yearly and whenever you change your insurance. You may click here to go to patient registration page. Please help us to assist you by promptly updating your insurance information prior to your first appointment of the year or your first visit with a different insurance policy, or you may complete the form at the office. Insurance companies require this annual authorization in order to process your claims.

    For patients with other health insurance, we can still serve your medical needs as an out-of-network provider, and payment in full is expected at the time of your service. It would be your responsibility to contact your insurance company's customer service department to find out if they offer any out-of-network coverage for your payment. We would provide you with the necessary medical coding information and payment receipt for you to submit the claim to your insurance company for reimbursement.

    For patients without health insurance or with healthcare savings accounts or high-deductible insurance plans, we would be happy to work with you to take care of your healthcare needs. We offer a competitive fee schedule for both the office consultation and for the lab tests, and payment in full is expected at the time of service.
