What to bring to your appointment:
• Your insurance card and co-payment
• New-patient registration form, if you have printed and filled it out at home
• A bag containing your current prescriptions or over-the-counter medications and supplements
• Name, address, and phone number of the local pharmacy and mail-order pharmacy.
• Pertinent information about your medical and surgical history
• Physician referral, if you are referred by another physician or other healthcare provider.

Doctor/Patient Relationships

Patient Responsibilities:
• To keep scheduled appointments (please read the office policies.)
• To be honest with the doctors and other healthcare workers
• To provide information about symptoms and conditions as clearly and briefly as possible
• To follow the treatment and care plan agreed upon
• To bring a list of the names and addresses of your other health care providers
• To bring to your appointments the medications (actual bottles) that you are currently taking
• To know what medications have been used unsuccessfully on you in the past
• To know what allergies you have (medications, vitamins, food or others)
• To provide your pharmacy name and number and your preference of 3- month or 1-month prescriptions
Prepare for your office visit by:
• Organizing your thoughts to make the most of your limited time
• Bringing along a friend or family member if you get tongue-tied in a doctor’s office
• Trying to be more efficient
Your time is just as valuable as your healthcare provider’s time. Following these simple guidelines and being prepared for your visits will not only make them more enjoyable but also will undoubtedly increase your level of care.
